I have worked on level building, level design and modeling on Leelh ,
Before modeling the ground, paint and placing objects , we do created with photoshop or on paper , a plan . « Where are loot ? , mobs ? , difficult passage , Zones , Important places , …. » after we can do worked on level building with « jardibot editor » ( a « home » vrml editor running in the browser )
